Need help with confirming your appointment? 

Head to the patient portal to confirm your appointment location, day & time!

If you do not have a patient portal account established already, please make sure to sign up via the link here

You will need to sign up for the portal using the same email address you booked for your eye exam, and you will need to activate the patient portal via your email address as well before logging in.
